New feature: Add bowling center

Hey Everyone,

I've added the ability for a user to add a bowling center to the database if it doesn't already exist. PLEASE DON'T ABUSE IT! If you can find your center in the dropdowns, use that instead of entering a new one.

When creating a stat sheet, if your bowling center (or city) doesn't show up in the dropdowns, then click "Add new center" link just to the right of the Centers dropdown box. A new form will open to allow you to enter the name, address and phone# of the center.

PLEASE DO ME A BIG FAVOR: When adding a bowling center, if you don't know the address and phone#, please look it up, call them, whatever. Entering all the correct information will greatly help me to confirm the center. Any centers that can't be confirmed will be removed from the database.

ANY ABUSE TO THIS FEATURE WILL RESULT IN BANNING!
